Current Job Openings
Registered Nurses (RN)
- Must hold an active state license.
- Home health experience is preferred, but we are willing to train.
- Responsibilities include assessing and evaluating client needs, collaborating with physicians, family members, and other healthcare professionals, and overseeing case management.
- Supervise LPNs and CNAs providing patient care.
- Must be a licensed driver or have reliable transportation to travel to client locations.
Licensed Practical Nurses (LPN)
- Must hold an active state certification.
- Experience in home health care is preferred, but training is available.
- Responsible for implementing the plan of care as outlined by the case manager.
- Monitor clients for changes and communicate with the healthcare team and physicians.
- Must be a licensed driver or have reliable transportation to travel to client locations.
Physical Therapists (PT)
- Must have the appropriate state license.
- Experience in home health PT services is preferred, but not required.
- Must be a licensed driver or have reliable transportation to travel to client locations.
Occupational Therapists (OT)
- Must have the appropriate state license.
- Experience providing OT services in the home is preferred, but not required.
- Must be a licensed driver or have reliable transportation to travel to client locations.
